Chronic Health Care

Chronic Health Care Plans, also known as GP Management Plans are available for our regular patients with ongoing medical conditions lasting more than 6 months. 


A Care Plan is a written plan between patient and GP to help best manage your medical condition. Including co-ordinating care with relevant allied health professionals. 

 

Medical conditions eligible for Care Plans include conditions such as:

 

  • Diabetes

  • Asthma

  • Arthritis

  • Cancer

  • Heart Disease

  • Osteoporosis

  • Or any other medical condition that has been present for more than 6 months


Once a Care Plan has been created you may be eligible to see an Allied Health Professional for 5 visits per year and receive a Medicare rebate for this service.

Allied Health Professionals include:

 

  • Physiotherapist

  • Dietitian

  • Podiatrist

  • Audiologist

  • Diabetes Nurse

  • Occupational Therapist

  • Pharmacist

  • Asthma Nurse

  • Exercise Physiologist

  • Psychologist

Care Plan Reviews 
Once a Care Plan is in place, it should be regularly reviewed by your GP. This is an 
important part of the plan cycle, to help achieve the goals and review results and make changes if necessary.

 

A Care Plan Review is usually under taken every 3 to 6 months.
